Virtual Consultation Services

Given the North East’s unique geography, the RAC-LBP program offers virtual consultations for residents in rural or remote areas or those facing travel difficulties. Assessments are available in both English and French.

Partnerships:

  • Direct referrals to Pain Clinics for Guided-Spine Injections
  • Direct referrals to HSN’s Integrated Chronic Pain Program
  • Direct referrals to EMG clinic/ Physiatrist and HSN’s Orthotics department
  • Direct referrals to Orthopedic Surgeons/ NEJAC for hip and knee consultations
  • Direct referrals to The Arthritis Society (TAS) for rheumatology consultations with APP

Renée Wilson is an Advanced Practice Physiotherapist at Health Sciences North (HSN) and the Practice Lead (PL) for the North East (NE) Low Back RAC, implemented in 2018. Renée is a clinical expert in managing low back conditions and leads by providing clinical oversight to nine Advanced Practice Providers (APPs) in the region. These APPs consist of five Physiotherapists and four Chiropractors serving a large geographical area (300,000 sq km) with a population of 557,000 residents, including 18.5% who identify as Francophone and 13.6% as Indigenous people.

I have been a physiotherapist since 2004 and the Clinical Practice Lead of the Ontario South East RAC LBP since 2018. I am based at Kingston Health Sciences Centre and we have an excellent team of 7 clinic based Advanced Practice Clinicians across our region to provide consultations close to a patient’s home. For surgical cases, we have an efficient referral pathway with our local spine surgeon sponsor, Dr. Julius Ebinu, and can coordinate referral to any of the other local spine surgeons. For lumbar radiculopathies < 12 months, we can refer for epidural steroid injections to the anesthesiologists at the HDH Pain Clinic or the interventionist physicians at KOPI as appropriate.

I am a big believer in promoting whole person health, interprofessional collaboration, and striving to improve health care efficiencies to optimize patient wellness, wait times, and outcomes. Aside from managing lumbar pain, I enjoy running, biking, ultimate frisbee, my dog, and adventures with friends and family.

Ravi Rastogi completed his Bachelor of Science in Physical Therapy from Western University in 1998. Ravi’s interest in research led him to return to Western University where he completed his Masters of Science in Physical Therapy in 2006. Since graduating Ravi has continued with an orthopedic specialization, working in the private sector and as a physiotherapist and clinical specialist in orthopaedics at London Health Sciences Centre (LHSC). Ravi credentialed in the McKenzie method of Mechanical Diagnosis and Therapy in 2011. Since 2018, Ravi has been practicing at LHSC in an Advanced Practice role as the lead of a Musculoskeletal Rapid Access Program for low back pain in Southwestern region of Ontario. Outside of work, Ravi’s interests include cycling and cycle touring as well as training for triathlons.

Meet Iriny Jacoub, Physiotherapist, Practice Lead for the Central Region Rapid Access Clinic for Low Back Pain at Oak Valley Health (including Markham Stouffville Hospital and Uxbridge Hospital) and mom of three. Iriny has advanced training in Spine Care, McKenzie training, manual therapy, concussion and vestibular therapy, acupuncture, Neuro-Developmental Treatment/BoBath, David Butler’s Neurodynamics, Pain Physiology and Occupational rehabilitation/WorkWell Functional Testing.

The Central Region Rapid Access Clinic for Low Back Pain covers a large area in the province and we have the largest group of Advanced Practice Providers (APPs) to be able to provide patients care close to home. The region encompasses South Simcoe, York region, West and Central North York to the Durham Border. We have developed several partnerships to expedite patient care including EMG/NCS tests, physiatry consultations, pain clinic referral that employ fluoroscopy as well as chronic pain management and with our local emergency departments and urgent community health clinics.

Ihab El-Sawaf is the Practice Leader and Manager of the RAC for Low Back Pain (LBP) in the Central East region. He is a licensed chiropractor that graduated from the Canadian Memorial Chiropractic College in 2002. He launched the RAC LBP program in 2019 and has worked at Scarborough Health Network (SHN), General site since the inception of the program. SHN is the regional spine surgical hub for the Central East area of Ontario. There are 18 Advanced Practice Professionals (APP) working with the RAC LBP in the Central East region. They are all licensed allied health professionals working in the Central East communities including: Scarborough, Durham, Northumberland, Peterborough and Kawartha Lakes.

The Central East RAC LBP has collaborative and working relationships with physiatrists and rheumatologist should the patient require consultation to non-operative specialists.

RAC LBP patients that require MRI imaging can be referred to the diagnostic imaging department of SHN by the Practice Leader.954
