Maria Rachevits is a Practice Lead for the Rapid Access Low Back Pain clinic Sunnybrook North Toronto region and an Advanced Practice Physiotherapist in the Hip and Knee Arthritis program at the Holland Center. She holds a Summa cum Laude medical degree from the Kiev Medical University (Ukraine) and a BSc (PT) degree from the University of Toronto (2003). She worked as an emergency physician in Bulgaria and Belgium before immigrating to Canada. She has twice won a grant provided by the European Union for a fellowship training in emergency medicine in Belgium. Currently she provides leadership to two Advanced Practice Providers in the RAC LBP model of care, has co-authored published papers on spine and hip/knee related research and has a few publications and delivered presentations on the topic of low back pain to specialized and general public audiences. She has received a PBR grant as a principle Investigator for “Telemedicine triage and Consultation in the Era of Pandemic” in 2022. Maria participated in the development of the Virtual Spine Assessment Toolkit by Bone and Joint Canada in 2021. She is a clinical instructor status only appointment at the Department of Physical Therapy, Faculty of Medicine at U of T.

Andrew Courchene is a physiotherapist and the Practice Lead for the RAC-LBP Mid-West and West Toronto region. Based out of University Health Network’s Toronto Western Hospital, Andrew and his team of 8 APPs provide timely access to low back pain care across the region. In addition to providing patients with comprehensive education and evidence based self management plans, the program has a close partnership with the Toronto Academic Pain Medicine Institute (TAPMI) for expedited injection therapy. Through collaborative workflows between practitioners in both programs, this partnership optimizes patient visits and decreases unnecessary follow-ups, benefiting both providers and patients. The TWH RAC-LBP program also has a partnership with the Spondylitis Screening Clinic at Toronto Western Hospital for early diagnosis and treatment of spondyloarthropathy. Finally, the team works closely with orthopaedic spine surgeons Dr. Raja Rampersaud and Dr. Christopher Neilson, along with neurosurgeon Dr. Taufik Valiante for patients requiring surgical consultation and management.

Henry Candelaria holds a Bachelor of Physical Health and Education from the University of Toronto. He graduated Magna Cum Laude from the Canadian Memorial Chiropractic College (CMCC) in 2007.

Henry has extensive expertise and experience managing a variety of musculoskeletal (MSK) injuries and conditions. His focus for the past 12 years has been on the diagnosis and management of spine related complaints, specifically those related to low back pain (LBP) and related conditions. He was one of the original ISAEC clinicians during the pilot program’s initial phase, and has been grateful to have continued to work within this program watching it morph and grow to what it is known as now, the provincial Rapid Access Clinic for Low Back Pain (RAC-LBP).

Henry is proud to work with excellent partners like the regional Central Intake Office and our front line Advanced Practice Providers (APPs) in the community, physiotherapists and chiropractors, who are the first contact for patients referred to the program. He is also proud of the relationships established with interventionalists, rheumatology, neurology, and spine surgical specialties to support patients when and if needed. In addition, the breath of community partnerships established through the program to support patients is also a point of pride for him.

Candelaria is confident that your experience within this program will help educate you on your condition, and in working with your primary care provider, will help you improve your ability to better manage it.

Jennifer Nugent, PT, DPT, CertMDT is registered physiotherapist in Ontario and Michigan with more than 20 years in experience in the advanced assessment of the spine. RAC-LBP is currently providing Advanced Practice assessments by 11 community providers in Sarnia (3), Petrolia(1), Watford (1), Tilbury (1), Essex (2), Leamington (1), and Windsor (2). We are looking forward to adding Chatham (2) in early 2025.

Windsor Regional Hospital is the neurosurgical hub for the Erie St Clair region of the Southwest Ontario Health Region. It is a Level B trauma center and Erie St.Clair Region’s centre for Cardiac Care, Stroke, Trauma, Cancer care and Neurosurgery (brain and spine). We have 5 neurosurgeons providing care and supporting the RAC-LBP : Erin Dyer, MD , Chief Neurosurgery, Balraj Jhawar, MD, Abdalla Shamisa, MD, Abdurrahim Elashaal, MD and Abdul Naeem, MD.

We have referral partnerships for expedited epidural and facet injections with WRH Interventional Radiology (Windsor), Erie Shores Healthcare, R Stapleton, MD (Leamington) and Bloor Pain Specialists (Etobicoke).
